Consultation
Your initial appointment is usually a consultation so that we may analyze your health history, diagnose your Surgical needs, and develop a treatment plan to best serve you. Also at this time our Financial Team will discuss all costs as related to your treatment. In some cases, your treatment can begin on the same day as the consultation. However, a more complex medical history or treatment plan may call for further evaluation or medical consultation. In these cases, treatment can usually begin on the second appointment.
Please bring the following to your initial appointment to expedite treatment:
- A list of ALL currently prescribed medications
- Any radiographs (X-rays) that you may have
- Referral from your Dentist
- If you have dental or medical insurance, please bring your card and any other insurance documents.
List of ALL medical conditions that may be of concern (Diabetes, High Blood Pressure, Taking Blood Thinners, Joint Replacements, Heart Conditions, Need for Antibiotic Pre-Medication)
Please note that
ALL patients under the age of 18 must have a parent or legal guardian present at the time of consultation.
If you are having IV Sedation/General Anesthesia, please have nothing to eat or drink eight (8) hours before your scheduled appointment time.
